Joining Club

Hi, 

This may be a silly question as I'm sure the answer is staring me in the face, but we wish to join the club, have the pdf to join, but I can't find any instructions as to where to send the form either.  I know there is an address listed and assume this where we post the form?  electronic option?  payment made to bank details listed on the form?

Am I missing the instructions/procedure/info and what happens after payment etc is made?
